Passive range of motion (PROM) exercises should be performed for the shoulder, emphasising gentle flexion, abduction, and external rotation, while active range of motion (AROM) exercises are encouraged for the elbow, wrist, and hand.
A reverse total shoulder replacement is a special type of shoulder surgery. During the surgery, a surgeon removes the damaged parts of the shoulder and replaces them with artificial parts. The shoulder joint is made up of the upper arm bone (humerus) and the shoulder blade (scapula).
Home Exercise Program after Reverse Total Shoulder Arthroplasty. Most patients using a home exercise program should perform their exercises 3-4 times per day. Stage 1: Begin immediately.

This protocol is intended to guide clinicians and patients through the post-operative course after a reverse shoulder arthroplasty. Specific interventions should be based on the needs of the individual and should consider exam findings and clinical decision making.
• Begin light resistive exercises • Theraband exercises to increase forward flexion, extension, and external rotation strength (limit IR strengthening at this point)
This protocol is intended to guide clinicians and patients through the post-operative course after a total shoulder arthroplasty (TSA) and hemiarthroplasty. Specific interventions should be based on the needs of the individual and should consider exam findings and clinical decision making. If you have questions, contact the referring physician.
Reverse Total Shoulder Arthroplasty (rTSA) is designed for the treatment of glenohumeral (GH) arthritis when it is associated with irreparable rotator cuff tears, complex fractures, severe glenoid deformity from dysplasia or wear, or for a revision of a previously failed conventional Total Shoulder Arthroplasty (TSA).
After reverse total shoulder arthroplasty (total shoulder replacement) surgery, patients are instructed to complete these post-operative exercises to help restore motion and flexibility in their shoulder.
Avoid light activities with wrist and hand for 2 weeks other than prescribed exercises. Expect return to moderate functional activities gradually at 3 months post-op. Expect return to more challenging activities (i.e. golfing and racquet sports) at 4-6 months.
DATES: Appointments. Physical therapy 2-3 x/week. Rehabilitation Goals. Promote soft tissue healing/maintain the integrity of the replaced joint. Restore AROM of elbow, wrist, and hand. Precautions. Sling x 6 weeks: only to be removed for exercise and bathing.
Pendulum exercises - no larger than a basketball, 5 minutes at time, alternate directions. Active ROM in elbow flexion and extension with the elbow at the side and humerus in neutral rotation. Forearm pronation and supination, finger and wrist ROM. Scapular retractions and trapezial stretch.
